My clownfish just jumped out of the tank, Why?

I also noticed that there was a worm-like animal swimming around. It was very small and it was spinning around. What was it?

My false clown jumped out of the tank too. I'm not sure why, but I think it might have to do with the fact that he was the only clown in the tank and they do best in groups. Although he was healthy, he was also acting weird, just hanging out in one corner all the time.

Then one day he disappeared, I looked all over the tank, under the rocks and around the outside of the tank, no clown. Then I found it two days later when I moved the couch and there he was, hardened onto the floor. Not cool at all.
